Coordinator’s Role

A supports coordinator is considered the quarterback of the supports team. They are a facilitator and liaison managing discussions focused on an individual’s needs. Supports coordination includes locating, coordinating, and monitoring services and supports for waiver participants.

They play a crucial role supporting people with intellectual/and or developmental disabilities by:

• Identifying appropriate services and supports.
• Advocating for the best interests of the participant based on his or her needs, preferences, and goals.

The supports coordinator assures Medicaid Waiver Requirements by:

• Monitoring health and welfare of the participant. vEnsuring individuals are receiving services in the ISP as outlined in the Waivers and AE Operating Agreement.
• Ensuring documentation is current.
